Lockout Roles and Responsibilties

WORKER(S)
*Note* For the purpose of this policy the term worker will also apply to any student involved in an apprenticeship, trades or industry related course of instruction. Students will work under the process of signing out locks for the duration of the maintenance or repairs. Once the work is completed the locks will be returned to the tool crib.

*Note* For the purpose of this policy the term worker will also apply to any contractor hired to perform repairs or maintenance where the contractor is not considered the prime contractor and the College is providing the work.

  1. The college requires all of its workers to be vigilant of their surroundings and to be aware of any change to their environment that might put them at risk.
  2. Any significant changes to the work environment should be reported to the supervisor or instructor.
  3. Workers are required to adhere to the site specific written lockout procedures for the area they are learning or working in.
  4. Locking out the equipment or device that they are working on. Workers are required to retain the keys to their personal locks that are locking out a machine for the duration of the work being performed.
  5. Workers shall never loan keys or locks to another worker nor will they transfer the responsibility of locking or unlocking to another worker.
  6. Workers will report any deficiencies of the procedures or the lockout program as a whole to their supervisor.

SUPERVISOR(S)

*Note* For the purpose of this policy supervisor means a person who instructs, directs and controls workers in the performance of their duties. Instructors are to be considered supervisors of students.

  1. Will be responsible for identifying devices or machinery that require lockout in their assigned work area(s).
  2. Will be responsible for developing site specific lockout procedures for their work area(s).
  3. Will be responsible for training workers in proper lockout procedures for their work area and associated tasks.
  4. Will supervise all workers performing repairs, maintenance or cleaning equipment or devices that require lockout. The supervision will include ensuring proper lockout procedures are adhered to.
  5. May be required to remove locks only under the following circumstances
    1. The worker that applied the lock is absent and every possible effort to contact the worker that applied the lock has been made and
    2. The supervisor is absolutely certain that the machinery or device can be operated safely prior to removing the lock.
  6. Will record any incidents of lockout violations and forward this information to the appropriate JOHSC or Worker representative, the Health, Safety & Environment Advisor (HSEA), and the appropriate Campus Administrator.
  7. Work in consultation with the HSEA in the development of future policies and procedures for the purpose of prevention.

HEALTH, SAFETY & ENVIRONMENT ADVISOR

  1. Will assist the Supervisor and/or Campus Administrators in developing, reviewing and/or adjusting procedures for lockout.
  2. Will keep records of incident and general observations for the period of two years.
  3. With the assistance of the appropriate campus Joint Occupational Health and Safety Committee (JOHSC) investigate any incidents of lockout violation(s).The investigating party will record any observations concerning the Lockout/Tag out Policy and provide copies to the Regional Health and Safety Committee.
  4. Along with the campus JOHSC, will annually review the Policy and established procedures for lockout and de-energization and take into consideration any suggestions provided by the JOHSC for improvement of the existing policy and applicable procedures.
  5. Will work with the Campus Administrator, worker supervisor, JOHSC or Worker Representative to investigate incidents and review the effectiveness of the procedures concerning lockout violations.

CAMPUS ADMINISTRATOR

  1. Will provide whatever assistance necessary to Supervisors, the JOHSC, or the HSEA to ensure effectiveness of the Lockout Program.
  2. Will provide whatever assistance necessary to an investigating party that is investigating a report of a lockout violation.

REGIONAL FACILITIES MANAGER

  1. Will ensure that all outside contractors not declared a prime contractor by a written agreement with the College are made aware of the Lockout/Tag out Policy and procedures.
  2. Will provide whatever assistance necessary to Supervisors, the JOHSC, or the HSEA to ensure effectiveness of the Lockout Program.
  3. Will provide whatever assistance necessary to an investigating party that is investigating a report of a lockout violation.

DIRECTOR OF HUMAN RESOURCES

  1. Ensure that Northern Lights College maintains an effective Lockout Program.
  2. Take remedial actions as necessary to sustain an effective Lockout program. These action may include discipline where required.
